Sounds like inferior steel, underbuilt assembly, or metal fatigue.

OURS BROKE LOOSE LAST WEDSDAY ON OUR DODGE DUALLY.. PULLING SINGLE OPEN TRAILER WE WERE LUCKY IT DIDNT TAER UP NOTHING NEITHER ..BUT IT BROKE APART IN THE SAME PLACE YOURS DID

Was by myself... which was good. The insurance adjuster came today and was really looking at the hitch. Just hope that cover everything like we paid for !!!
