Default centerforce dual friction clutch

What would cause the disk hub to brake in a dual friction clutch. It's behind a 375-400 horse sbc that is in a 2600lb 34 ford pick-up. The clutch has less than 10000 miles on it. Has anybody had this happen to them.

I'm guessing but it could be a misaleigned bell housing or bad pilot bearing.
